Maplewood offers round-the-clock nursing care provided by registered and licensed nurses in conjunction with physicians on staff at neighboring hospitals and clinics. Alongside our dedicated nurses and physicians is a staff of hard working, well trained nursing assistants, licensed social workers, activity professionals, physical, speech, and occupational therapists. We also have a wide array of specialists from the Sauk Prairie area that come to Maplewood including: psychologists, podiatrists, dentists, audiologists, optometrists, and hospice care givers.

Saint Jude Hospice

Saint Jude Hospice is unique in that we allow for God’s love to freely flow. Rooted in our Christian faith, our philosophy is simple: “radical, loving care.” Love is what drives Saint Jude’s to provide an environment of generosity and kindness, of honor and dignity, and all within the comfort of the home. During a season of vulnerability and loss, Saint Jude’s approach to care is reassuring, ushering in the comforting peace of Christ while leaving space for unexpected moments of joy.

The mission of Saint Jude Hospice: to bring healing to those with life-limiting or terminal illness, who—along with their healthcare provider—have decided to seek comfort care over a cure. We carry out this mission in each of our metro locations serving, Madison and Central Wisconsin.. No one will be turned away because of an inability to pay; funding is provided by Medicare, Medicaid, private insurance and even individuals.

Home Health United (HHU)

Home Health United (HHU) offers an integrated and comprehensive variety of community home health services and products in southern Wisconsin, including home health, hospice, home medical equipment, community health services, and immunization clinics. We’ve been “Your Community Hospice for Over 20 Years”! We are dedicated to helping people remain as healthy, safe and independent as possible in their own home setting.

HHU is a nonprofit organization and United Way agency. We are Medicare/Medicaid certified and accept a wide variety of health plans.

Our team of professionals offers a range of services to meet almost any home health care need from pediatrics to geriatrics. Our more than 450 employees have been carefully screened and trained so patients and their families will feel comfortable, safe and secure as they provide care in the home.

Heartland Hospice

Heartland Hospice supports patient and family wherever they are in their health care journey and upholds the patient’s and family’s wishes, goals and values. Patients needn’t lose hope nor stop comforting treatments to receive Heartland Hospice services. Advance directives and health care decisions do not need to be completed before receiving Hospice care. We encourage and can facilitate open discussions that foster the understanding of a loved one’s health care choices, while supporting the patient and family during these challenging circumstances.

Heartland strives to enrich life through the skills, understanding and compassion of our team members. We build a unique partnership with the patients and their loved ones which helps them throughout their health care journey. Our team includes: Physicians, Registered Nurses, Social Workers, Hospice Aides, Spiritual Care Coordinators, Bereavement Counselors, and Volunteers. We also offer PT, OT, and Speech Therapies along with Massage, Music, Pet and Art Therapy.

Our caregivers help manage pain and symptoms, enabling our patients to live their lives as fully and comfortably as possible. Social workers and Spiritual counselors create a comforting environment in which patients and families are able to share their thoughts, hopes and concerns. And special bereavement programs provide emotional support to patients and families who need help coping.

Southern Care

Agrace

For 35 years, Agrace has been a nonprofit, community based hospice and palliative care agency dedicated to providing expert physical, emotional and spiritual support to patients and families dealing with serious illness and we are proud to partner with Maplewood Sauk Prairie.

Bringing comfort to patients with life-limiting conditions, and their families, is more than a mission for Agrace—it’s a way of delivering care that addresses the needs of the whole person at a significant time of life. Whether comfort takes the form of physical, emotional or spiritual care, our interdisciplinary team is ready to provide comprehensive hospice services to every person in our community who needs and wants them.

When in-home care cannot meet the needs of our patients and families, short-term inpatient care at the Agrace campus in Madison or contracted hospitals and skilled nursing facilities may be an option. Inpatient care provides short-term, intensive monitoring and treatment of acute pain or other complex symptoms. Agrace also recognizes the need to provide families a break from ongoing caregiving by offering respite care.

For those not eligible for hospice, Agrace provides in-home palliative care consultations to identify the medical and emotional issues that need to be addressed to improve the patient’s quality of life. Agrace Care Navigation helps people manage a serious illness more confidently at home with guidance and education.
